Provider Enrollment Eligibility and Reenrollment
Providers have three (3) options regarding applications for Health First Colorado (Colorado's Medicaid Program).
- Enrollment: Enrollment is the process by which an individual or entity that has never been enrolled as a provider submits a new provider enrollment application for participation in the program. Visit the Provider Web Portal Enrollment web page to start an application.
- Revalidation: Revalidation is the process by which an individual or entity actively enrolled as a provider submits a revalidation application and receives approval to continue participation in the program. Revalidation is required at least every five (5) years. Providers may begin the revalidation process up to six (6) months prior to the revalidation due date. Providers also have the revalidation link available 6 months after the revalidation date and should not attempt to re-enroll during this period. Log in to the Provider Web Portal and click Revalidation to get started.
- NEW: Reenrollment: Providers that were previously enrolled in the program and were later disenrolled may reenroll. Log in to the Provider Web Portal and click Reenrollment to get started if it has been past 6 months from the revalidation date.
