Registering your personal information is the first step to gain access to the Provider Portal. Once you register your personal information, you will then enter security information, such as a user ID and password, this will help ensure your information is protected and available only to you or those authorized to see it.
Registering Personal Information
1. | Enter your first name and last name. |
2. | Enter your date of birth in MM/DD/CCYY format, or select the calendar icon and select the date. |

3. | Enter the zip code for your address. |
4. | Enter the appropriate data in all remaining required fields. These values are used to validate whether or not you have previously registered. |
5. | Click Continue or press Enter to submit your personal information. |
If your registration is successful, you will receive a confirmation message and you will be taken to the next step in the the registration process. | |
If you have previously registered, you will receive a message stating that you are already registered and you will be directed to log in. |
