Registration Step 1

Registering your personal information is the first step to gain access to the Provider Portal. Once you register your personal information, you will then enter security information, such as a user ID and password, this will help ensure your information is protected and available only to you or those authorized to see it.

Registering Personal Information

1. Enter your first name and last name.
2. Enter your date of birth in MM/DD/CCYY format, or select the calendar icon and select the date.

Note: Use the back arrow to scroll back through the year.

Note: Information required in the first two steps may vary, depending on your role.

3. Enter the zip code for your address.
4. Enter the appropriate data in all remaining required fields. These values are used to validate whether or not you have previously registered.
5. Click Continue or press Enter to submit your personal information.
If your registration is successful, you will receive a confirmation message and you will be taken to the next step in the the registration process.
If you have previously registered, you will receive a message stating that you are already registered and you will be directed to log in.
